This screenshot shows step one of the Oracle ASM Support Workbench Create Package wizard in Oracle Enterprise Manager.

On this page of the wizard, you are required to provide a name for the package. You can optionally enter a package description, select whether to send the package to Oracle Support, or create a new service request.

To send the package to Oracle support, you need to provide your My Oracle Support user name, password, and customer support identifier.

There are Cancel and Next buttons to either cancel the wizard or proceed to the next page of the wizard.
