Template Name (required)
Redundancy (High, Mirrored, or Unprotected levels) option buttons
Striped (Fine or Coarse granularity) option buttons
Extent Regions
Primary: Hot or Cold
Mirror: Hot or Cold
On the right side of the page are Show SQL, Cancel, and OK buttons.
